Nurse-Family Partnership
Nurse-Family Partnership (NFP) is a free program that supports first-time moms build healthy futures for themselves, their babies, and their families. Through regular visits with a specially trained Public Health Nurse (PHN), moms receive the support, information, and encouragement they need, starting in pregnancy and continuing through their child’s second birthday.
Together, they build a strong, supportive relationship. The PHN becomes a trusted guide, helping moms and families care for their babies, set goals, and take steps toward a brighter, more secure future for their family.
For more than 40 years, research has consistently shown that NFP helps keep children healthy and safe while improving long-term outcomes for moms and their families.

What a Participants Can Expect from Nurse-Family Partnership
Visits with a specially trained Public Health Nurse occur every 2 to 3 weeks, or on a schedule that fits individual needs.
Support is provided on topics such as pregnancy health, parenting, personal goals, child development and behavior, and accessing helpful services.
Services begin during pregnancy and continue until the child’s second birthday.
Scheduling is flexible, with visits arranged at times that work best.
Most visits take place in the home, though other locations or virtual options may be available.
Each visit typically lasts about 60 minutes and may include basic health checks, such as weight and blood pressure.
Participation is always voluntary.
Who is Eligible?
First-time moms (28 weeks gestation or less)
Ventura County resident
Facing social, financial, or environmental challenges that may affect health or well-being
How to Enroll in NFP
Anyone can refer to NFP. Pregnant individuals who meet eligibility requirements can refer themselves or be referred by a healthcare provider or community agency via the CHN referral form (scroll down to bottom of linked page).
After a referral is received, they will be screened and assessed for NFP eligibility. If someone is not eligible, they will be offered information about other home visiting and support programs in Ventura County.
