data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>Nurse-Family Partnership (NFP) is a free program that supports first-time moms build healthy futures for themselves, their babies, and their families. Through regular visits with a specially trained Public Health Nurse (PHN), moms receive the support, information, and encouragement they need, starting in pregnancy and continuing through their child\u2019s second birthday.<br \/><br \/>Together, they build a strong, supportive relationship. The PHN becomes a trusted guide, helping moms and families care for their babies, set goals, and take steps toward a brighter, more secure future for their family.<br \/><br \/>For more than 40 years, research has consistently shown that NFP helps keep children healthy and safe while improving long-term outcomes for moms and their families.<br \/><br \/><strong>NFP Goals<\/strong><\/p><ul><li><p>Support healthy pregnancies<\/p><\/li><li><p>Nurture strong early health and development for children<\/p><\/li><li><p>Strengthen long-term opportunities for families<\/p><\/li><\/ul><p><strong>What a Participants Can Expect from Nurse-Family Partnership<\/strong><\/p><ul><li><p>Visits with a specially trained Public Health Nurse occur every 2 to 3 weeks, or on a schedule that fits individual needs.<\/p><\/li><li><p>Support is provided on topics such as pregnancy health, parenting, personal goals, child development and behavior, and accessing helpful services.<\/p><\/li><li><p>Services begin during pregnancy and continue until the child\u2019s second birthday.<\/p><\/li><li><p>Scheduling is flexible, with visits arranged at times that work best.<\/p><\/li><li><p>Most visits take place in the home, though other locations or virtual options may be available.<\/p><\/li><li><p>Each visit typically lasts about 60 minutes and may include basic health checks, such as weight and blood pressure.<\/p><\/li><li><p>Participation is always voluntary.<\/p><\/li><\/ul><p><strong>Who is Eligible?<\/strong><\/p><ul><li><p>First-time moms (28 weeks gestation or less)<\/p><\/li><li><p>Ventura County resident<\/p><\/li><li><p>Facing social, financial, or environmental challenges that may affect health or well-being<\/p><\/li><\/ul><p><strong>How to Enroll in NFP<\/strong><br \/><br \/>Anyone can refer to NFP. \u00a0Pregnant individuals who meet eligibility requirements can refer themselves or be referred by a healthcare provider or community agency via the CHN <a href=\"https:\/\/hca.venturacounty.gov\/public-health\/community-health-nursing\/\">referral form<\/a> (scroll down to bottom of linked page).<br \/><br \/>After a referral is received, they will be screened and assessed for NFP eligibility.
