data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p><strong>Purpose<\/strong><\/p><p>Cal\u2013Learn is a statewide mandatory program for pregnant and parenting teens, both moms and dads, who are on cash aid or CalWORKs. The goal of Cal-Learn is to help teens graduate from high school, become independent and form healthy families.<\/p><p><strong>Goals<\/strong><\/p><ul><li>Increase social and emotional support<\/li><li>Improve health birth outcomes<\/li><li>Improve pregnancy planning and spacing<\/li><li>Increase educational attainment- School graduation and educational or vocational advancement<\/li><li>Increase access to and utilization of needed services<\/li><\/ul><p><strong>Services <\/strong><\/p><ul><li>Assessment to determine what services and support client needs to meet educational goals.<\/li><li>Develop a plan with client to receive services they need, including childcare, to enroll in school or stay in school.<\/li><li>Take action to put client on the road to reaching their goals.<\/li><li>Home visits and tracking, meeting 2x a month, to ensure client is making progress toward their diploma or equivalent and meeting their goals.<\/li><li>Periodic plan review to identify what works and what doesn\u2019t.<\/li><li>Provide education on available resources.<\/li><\/ul><p>Our Public Health Social Workers focus on positive youth development by building strengths, values, goals, and resilience. They promote the development of collaboration between integrated systems of care to support clients during pregnancy and parenthood, supporting their children, and aid clients to be self-sufficient and lead healthy lifestyles and make healthy lifestyle choices.
