Prevention Program (CDPP) offers multiple services focused on the prevention and control of chronic diseases such as prediabetes, type 2 diabetes, high blood pressure, and heart disease, as well as opportunities for physical activity including free Zumba\u00ae and Seated Tai Chi classes for adults. Trained health educators and lifestyle coaches provide education, information, and resources to individuals, families, and community groups to equip them with important skills needed to lead healthier lifestyles that are critical in preventing chronic disease and improving health and wellness. Referrals and resources to health care and social assistance programs are provided according to individual needs.<\/span><span
