you must register the birth at the Office of Vital Records.\u00a0 Registration is completed by appointment only.\u00a0 Our office can register the birth if it took place within 1 year of the date of birth.\u00a0 Out of Hospital Births that took place more than 1 year from the date of birth will need to be registered with the California Department of Public Health Vital Records Section as a Delayed Registration of Birth\u00a0(<a href=\"https:\/\/www.ochealthinfo.com\/sites\/hca\/files\/2021-11\/Delayed_Registration_of_Birth_Pamphlet_%2812-21-2020%29.pdf\" target=\"_blank\" rel=\"noopener\">CDPH Delayed Registration of Birth Instruction<\/a>)\u00a0or court ordered delayed registration of birth\u00a0(<a href=\"https:\/\/www.ochealthinfo.com\/sites\/hca\/files\/2021-11\/Court_Order_Delayed_Pamphlet_ADA_%2812-30-2020%29.pdf\" target=\"_blank\" rel=\"noopener\">CDPH Court Ordered Delayed Registration of Birth Instructions<\/a>).<\/p><p><strong>How to register an Out of Hospital Birth at the Office of Vital Records:\u00a0<\/strong><\/p><ol><li>Review and complete the <a href=\"https:\/\/hca.venturacounty.gov\/public-health\/wp-content\/uploads\/sites\/3\/2024\/03\/Home_Birth_Packet_Rev-Jan-2024.pdf\" target=\"_blank\" rel=\"noopener\">Out of Hospital Birth Instructions Packet<\/a>.<\/li><li>Review and complete the <a href=\"https:\/\/hca.venturacounty.gov\/public-health\/wp-content\/uploads\/sites\/3\/2024\/11\/Out-of-Hospital-Checklist_11-2024.pdf\" target=\"_blank\" rel=\"noopener\">Out of Hospital Birth Checklist<\/a> with list of instructions.<\/li><li>Email <a
