following information is intended for the <strong>general public<\/strong>. If you are a funeral director and need to order Death Certificates\/pay for a Burial Permit, please refer to the section labeled &#8220;<a href=\"https:\/\/hca.venturacounty.gov\/public-health\/vitalrecords\/for-funeral-directors\/\" data-uw-rm-brl=\"PR\" data-uw-original-href=\"https:\/\/hca.venturacounty.gov\/public-health\/vitalrecords\/for-funeral-directors\/\">General Information for Funeral Establishments Only<\/a>&#8221; in the menu on the left.<\/p><p><strong>The fees for certified copies are listed below, and this fee is non-refundable. If a record is not found, a Certificate of No Record will be issued.<\/strong><\/p><p><strong>To request in person:<\/strong><\/p><ul><li>Print and complete parts 1,2, and 4 of the application. Part 3 must be completed in the presence of the Office of Vital Records Staff.<\/li><li>Bring your completed application, payment, and a valid government-issued ID to our office during regular business hours.<\/li><li>We accept Visa, Mastercard, checks, and cash; we do not accept $100 bills.<\/li><\/ul><p><strong>To request by mail:<\/strong><\/p><ul><li>Indicate the number of certified copies you are purchasing.<\/li><li>Include payment in the form of a personal check, postal, or bank money order made payable to Ventura County Vital Records.<\/li><li>Mail orders paid by personal check are held for 14 working days.<\/li><li>Include a notarized sworn statement.<\/li><li>Mail the application with the appropriate fee and a self-addressed, stamped envelope to:\u00a0<\/li><\/ul><p style=\"text-align: center;\"><strong>Ventura County Vital Records<\/strong><br \/><strong>2240 E.
