program serves transitional age youth (TAY), ages 18-25, who are diagnosed with a serious mental illness, who may have a substance use disorder and who are at risk of homelessness, incarceration or hospitalization. This program provides supportive and social rehabilitation services to promote independent living and successful community integration throughout the county.<\/p><p>The program supports young adults in moving toward personal recovery by providing stabilization and skill development to live an independent and successful life within the community.
