long-term commitment to transforming and strengthening Medi-Cal, making the program more equitable, coordinated, and person-centered to help people maximize their health and well-being.<\/p><p><strong>Who Is Eligible?<\/strong><\/p><p>Gold Coast Health Plan members who are experiencing one of the following may be eligible and are encouraged to request a referral:<\/p><ul><li>Homelessness<\/li><li>Serious mental illness or substance use disorder<\/li><li>Frequent hospital or ER visits<\/li><li>Recent incarceration<\/li><\/ul><p><strong>How To Get a Management<\/strong><\/p><p>Managed Care Plans that already provide medical coverage for low-income Californians, are now able to provide access to other necessary supports to improve the health and well-being of Medi-Cal members including <strong>Enhanced Care Management<\/strong>, and <strong>Community Support<\/strong> for help with other social services such as housing and food.<\/p><p>To
